Important Information for Parents/Guardians regarding

Important:

Please notify the school if your child is absent due to a serious illness or communicable disease. A physician's note is required for a student who has recovered from a communicable disease and is returning to school. The student must be seen by the school nurse before a final determination is made.

The exclusion times listed below are the minimum days a child should remain out of school for a particular illness. The school will honor the private physician's recommendation for additional exclusion days.

Communicable Disease Exclusion from School

Chicken Pox six days after outset of rash and/or until lesions are dry.

Physician's note is required.

Common Cold should pot attend school if student has a temperature.

Conjunctivitis (pink eye) until therapy has been instituted for 24 hours and discharge has ceased. Physician's note is required.

German Measles (Rubella) five days after appearance of rash. Physician's note is required.

Hepatitis Infection until end of febrile period and/or seven days following the onset of jaundice. Physician's note is required.

Impetigo until 24 hours after treatment begins. Physician's note is required. Area must be covered in school.

Measles (Rubeola) from onset of symptoms until rash and other symptoms are gone. May be five to ten days. Physician's note is required.

Meningitis, Meningococcal until note is received from physician stating patient is able to return.

Mononucleosis exclusion not necessary.

Mumps for acute illness and/or nine days after swelling appears.

Physician's note is required.

Ringworm Physician's note required. Area is to be covered in school.

Scabies until the day after treatment. Physician's note is required.

Streptococcal Infection: 24 hours after beginning of antibiotic therapy. Physician's

Sore throat, Scarlet Fever, note is required

Undiagnosed rash physician's note is required, stating patient is able to return to school.

Health Guidelines

These policies and guidelines were designed to help protect the health of our students and staff. Please feel free to contact the School Nurse’s office with any questions or concerns.

All absences and late to school arrivals should be reported to the Main Office or the Nurse's Office (973)-838-6390 by 8:30 AM.
In accordance with Bloomingdale Board of Education Regulation 5330, the administration of medication-- including over the counter medication--during school hours requires a completed Physician and Parent/Guardian medication authorization form. Medication to be administered during school must be brought to the Nurse’s Office by the parent/guardian in the original prescription bottle or original container. Medication authorization forms are available at this link or in the Nurse’s Office.
As per N.J.A.C. 8-57-4: Immunization of Pupils in School, students must meet the minimum immunization requirements for school attendance in NJ. Please note that all 6^th-grade students must show proof of Tdap (Adacel, Boostrix) and Meningococcal (Menactra, Menveo) vaccination. If students are not in compliance with immunization requirements, a Religious Exemption, Medical Exemption, or Provisional Admittance Request must be on file with the School Nurse. Exclusion from school based on failure to comply with immunization requirements will be enforced.
Please notify the School Nurse when a student has a brace, cast, needs to use crutches and/or needs to be excused from Physical Education or sports. A doctor’s note must state the medical equipment prescribed, reason for PE/sports excuse and the length of time treatment is necessary.
Students with a temperature of 100° or above will be sent home due to fever. Students are expected to remain fever free without the use of fever-reducing medication for 24 hours before returning to school.
If a student vomits during the night or in the morning before school, please do not send him/her to school. If a student vomits in school he/she will be sent home.
Students with a confirmed case of strep throat must be on medication (antibiotic) for at least 24 hours before returning to school. A doctor’s note stating that treatment has been started is required for return to school
Please notify the School Nurse if head lice or nits are found in a student’s hair.
Students with a suspected case of Conjunctivitis will be immediately referred to a health care provider for evaluation. Students with a diagnosed case of Conjunctivitis (pink eye) should be on medication for at least 24 hours before returning to school.
Students with an unidentified rash or skin eruptions will be referred to a health care provider for evaluation. A physician’s note stating that the rash or skin eruption has been identified, is not contagious, and is being treated (if necessary) is required for return to school.
Students with Chicken Pox should be excluded from school until open sores are scabbed over and dry.
Students with ringworm require a physician’s note stating that treatment is in progress. The affected area is to be covered while in school.
