Sports Physical Forms

• History Form (Completed by Parent)
  • Must be completed by the parent regardless of whether a family physician or the school physical conducts the exam
• Athlete Physical Evaluation History Form (Completed by Doctor) 
  • Use this form if you have your healthcare provider complete the physical. This form is to be completed by your physician.
• Consent for the School Physician complete the physical.
  • Use this form if you consent to have the school physician complete the physical.
